If you need information from CHCS that isn't readily available through standard reports, then a CHCS Ad Hoc is the way to go.

  1. Don’t “recreate the wheel”.  Review existing CHCS Ad Hoc's that are available on this website for immediate implementation (coordinate installation with your site’s CHCS systems office).  Submit a request to view/download existing CHCS Ad Hoc.


  2. If you are unable to locate an existing CHCS Ad Hoc or need a modification to an existing CHCS Ad Hoc, please submit a request for new CHCS Ad Hoc support.*


  3. *  If you need assistance with a site developed CHCS Ad Hoc, submit a CHCS Ad Hoc request form and include a listing of the CHCS Ad Hoc templates in either the “Next, …” or the “Now, …” input box.
